The use of online forms was discussed at the corporation discussion meeting last night. It was brought up that 20% of forms turned in are still paper. This is something we can probably get better at here at LJH. You can access all forms by clicking here: ONLINE FORMS. If you are having trouble with an online form, please put in a technology order or call Chris McCann at the tech office for assistance.
